Hotel Reservations: To secure our special discount rate, reservations must be made through the hotel 30 days prior to the seminar. Group name: “Spine Research Institute of San Diego.”
Pre-registration requirements: Payment required via credit card by phone, or with a check postmarked at least four weeks prior to the seminar. Within four weeks, a $25 minimum late fee will be added. Walk-ins will be charged a $50 minimum at door fee in addition to the pre-registration fee.
§ Prerequisite: All four modules. Leads to Advanced Certificate of Competency from SRISD in Whiplash Injury Biomechanics & Traumatology.
SRISD Cancellation Policy: A $50.00 administrative fee will apply to any cancellation. Refunds are not available for cancellations made within 7 days of the seminar date. However, attendees may apply these funds towards any alternative seminar date within 12 months. Credit may not be applied beyond that 12-month period.
